Prior sampling considerations

In the environment and in the hands of technicians responsible for sampling, saprophytic microorganisms are present and that may lead to a misinterpretation of cultures due to improper handling of instruments. Hence the importance of using maximum measures of asepsis as well as an established order for sampling, avoiding as much as possible, the accidental contamination of the plates and mistakes in labeling the place where the sample has been obtained.
For all this, it is important to consider the following steps:

- Sterilization of media and culture media used.
- Disinfection of the sampling equipment.
- Hand washing and use of sterile gloves.
- Form for data collection and label the plates with the appropriate culture medium and following the pre-established.
- After each sample, seal the plates to avoid contamination in situ.
- Disinfection of the machine head with sterile gauze soaked in alcohol 70%. Then allow drying before the next sample.

Sampling
----------------------------

The samples shall be implemented before the start of the surgical activity with the lowest possible number of people present, without opening doors or moving any object while performing the sampling.
Sampling must be done in two areas of the operating room; one in the grid of impulsion of the conditioned air, to evaluate the state of the system of the air. And another one an approximated meter above the ground in the center of the operating room, in order to evaluate the system of the air, the state of hygiene and cleaning of the operating room.
Time and volume of sampling depend on the suspected environmental contamination that:
· White rooms, sterile zones, operating rooms, laminar flow cabins, air in air conditioning systems: 0,5 l/s (30 l/min.). · Clean zones
Transport:
The plates, once gathered the sample, will be sealed around with "parafilm" to avoid the movement of the lids and its possible accidental pollution, and will have to be transported refreshed (2-4 ºC) and protected from the light. In addition, plates will have to be labeled with the date and hour of sampling.

It is necessary to warn the laboratory of the sending of these samples and indicate the analysis that is requested.

As key data for fungi recount, it would be admissible values <4UFC / m³, whereas for aerobic mesophilic flora the total allowable values are:

<10 CFU / m³.
Clean environment:
10-100 CFU / m³.
Acceptable environment:
100-200 CFU / m³.

Pollution levels are expressed as colony-forming units per cubic meter of air, CFU / m³.
For interpretation, it should be noted that the ideal would be to have 0 CFU / m³, but can not guarantee absolute sterility in the operating room and is therefore not uncommon to have 1-2 CFU / m³.

It is common to have counts 2 to 3 times higher in samples collected in the center of the room than the samples obtained from the air inlet.
