In healthy individuals after administration of a small evening dose of dexamethasone, at 8 am of the following day a maximum inhibition of cortisol is observed. In patients with Cushing's syndrome this effect is not observed.
A cortisol concentration in serum above 50 nmol/L provides a sensibility in diagnostic of Cushing's syndrome very close to a 100%. The method of analysis used for the determination has little influence on this value.
This test has between 10% to 15% false positives which might be due to:
a) enzyme inducer treatment (phenytoin, phenobarbital, rifampicin), b) estrogen or tamoxifen treatment, and c) severe acute illness, alcoholism, major depression, etc. The false negative response rate is 2%.
